Eligibility Waiting Period for Coverage:

  1. None – if you are working for the employer on the policy effective date; or
  2. The first day of the month following the date you were hired – if you start working for the employer after the policy effective date.
BASIC LIFE INSURANCE (EMPLOYEE)
Guaranteed Issue Amount Maximum Amount:
$100,000 1 times your annual earnings, subject to a maximum of $200,000 rounded to the next higher $1,000 if notalready a multiple of $1,000. However, in no event will your basic amount of life insurance be less than $10,000.
SUPPLEMENTAL LIFE INSURANCE (EMPLOYEE)
Guaranteed Issue Amount Maximum Amount:
$100,000 The amount you elect in increments of $10,000, subject to the lesser of $350,000 or 3 times your annual earnings. However, in no event will your supplemental amount of life insurance be less than $10,000.
DEPENDENT LIFE INSURANCE BENEFIT: SUPPLEMENTAL AMOUNT OF DEPENDENT LIFE INSURANCE
Guaranteed Issue Amount Maximum Amount:
Spouse $25,000 The amount you elect in increments of $5,000, subject to a minimum of $5,000 and a maximum of $50,000
Dependent children: age 15 day(s) but under age 6 month(s) $500
Dependent children: age 6 month(s) but under 25 year(s) $10,000
The amount of spouse supplemental coverage may never exceed 50% of the basic amount of life insurance in force for the employee
ADDITIONAL ACCIDENTAL DEATH AND DISMEMBERMENT BENEFITS(EMPLOYEE)

$100,000 The amount you elect in increments of $10,000, subject to the lesser of $350,000 or 3 times your annual earnings. However, in no event will your supplemental amount of life insurance be less than $10,000.
SHORT TERM DISABILITY
LONG TERM DISABILITY : ELIMINATION PERIOD: 180 DAY(S)
  • Maximum monthly benefit: $5,000
  • Minimum monthly benefit: the greater of
    1. $100; or
    2. 10% of the benefit based on monthly income loss before the deduction of `other income benefits.
  • Benefit percentage: 50%

HSA Take charge of your health care spending with a Health Savings Account (HSA). Contributions to an HSA are tax free and withdrawals for qualified medical expenses are tax free. Your HSA can be used for qualified expenses, including those of your spouse and/or tax dependent(s), even if they are not covered by your plan. If you are not enrolled in a HDHP but you have unused HSA funds from a previous account, those funds can still be used for qualified medical expenses.

Each year, the IRS places a limit on the maximum amount that can be contributed to HSA accounts. For 2019-2020, contributons (wh,h include any employer contribution) are limited to the following:
